We’re committed to making healthcare affordable for everyone, regardless of financial situation. Our Sliding Fee Discount Program helps families and individuals get the medical, dental, behavioral, and women’s health services they need at reduced rates based on family size and income.

Patients who qualify also receive lab services at no additional cost, even when multiple tests are required. If you have private insurance, the lab will bill your provider, but you won’t be responsible for any unpaid balance.

We make applying for affordable care options easy, and our dedicated team is available to help you along the way. To apply, you will need to provide us proof of all income you earned in the past 30 days.

Applications need to be renewed annually, or every 6 months if you provide a letter of support. Our friendly staff will guide you through the process, ensuring your family gets the care they deserve.
