We’ve made it easy to become a new patient at HCHC. You can register as a new patient by calling, visiting any of our clinics, or through our convenient Patient Portal. Just click the “Pre-Register” button to provide a few details about you. Once your information is submitted, our team will reach out to you to schedule your first appointment.

At HCHC, we’re committed to making your healthcare experience as smooth and stress-free as possible. From what to bring to your visits, to affordable payment options, and ensuring timely appointments, we’ve got you covered every step of the way.

What to Bring

For every visit, please bring your insurance card and a photo ID. If any of your information has changed, let us know when you arrive. Each year, we’ll ask you to complete updated registration forms to keep your records current. We’ll also take your photo at your first visit to ensure accurate identification at future appointments. And don’t forget to bring a list of all medications, including prescriptions, over-the-counter, and those from other providers.

If you need to reschedule, no worries—just give us a call at least 24 hours in advance, and we’ll be happy to adjust your appointment. We are committed to removing barriers to healthcare, so if you need help with transportation, let us know. Our Community Resources Coordinator can help connect you with the resources you need.

Medications & Refills

A Note About Prescriptions.

Your health is our priority, and we follow these guidelines to ensure you received the safest care:

  • We do not provide long-term pain management or prescribe daily pain medications, but we will connect you with a pain management specialist if needed.
  • For chronic conditions like high blood pressure or diabetes, we’ll provide enough refills for 30 or 90 days during your visit. Make sure to schedule a follow-up when you’re down to a 30-day supply to avoid running out.
  • To keep you safe, we don’t call in new prescriptions or refills without an appointment. However, in urgent cases, our on-call physician can approve a short refill to hold you over until your next visit.
