HCHC’s remarkable team of providers, support staff, and non-medical and medical case managers work together seamlessly to provide comprehensive, stigma-free care if you are living with HIV/AIDS. Our unique areas of expertise, perspective, and skills provide a holistic care environment to help you not only survive with your chronic disease but thrive.

HIV/AIDS awareness is a sensitive and vital topic for communities. Knowing your HIV status is critical to keep yourself and your partner(s) healthy. We offer confidential, rapid HIV Testing at our Harrisonburg offices and other sites throughout the community.

Case Management

Client-Centered Case Management

Your medical case manager will help you develop an individualized service plan focused on holistic health and well-being, not just your HIV/AIDS diagnosis. Working towards better health and self-sufficiency, your case manager will assist you in accessing specialized treatment for HIV/AIDS, primary medical care, and dental care, and can also help you access mental health services, substance abuse treatment, nutritional counseling, and any other referred medical service. Your will also receive help navigating complex health insurance systems to ensure timely access to your treatments and medications.

Housing Assistance

Housing is a basic human need crucial to maintaining one’s health. However, sometimes, it can be difficult to access and maintain adequate housing. HCHC’s Case Managers will work with you to find the best solution to your housing needs, whether it be through a Section 8 voucher, subsidized housing, working on budgeting issues, or transitioning out of a shelter. We can assist with rent, utility payments, and security deposits in short-term emergencies and run a long-term, subsidy-based housing program for eligible clients.

Food Pantry

Food insecurity is an issue that affects thousands of Americans. Accessing nutritious foods including fresh fruits and vegetables can be particularly challenging when living on a reduced income. In order to meet the nutrition needs of our clients, Care Management provides an on-site food pantry and also delivers food directly to clients’ homes. Thanks to grant funding and local donations, we are able to stock our food pantry with nutritious foods, paper products, and nutritional supplements.

An HIV diagnosis can be overwhelming, but you can live a long and healthy life with the disease with proper management and sticking to your personalized treatment plan. As a Ryan White Program grantee, our Care Management team makes quality care and appropriate treatment available to low-income individuals living with HIV.

To be a Care Management client, you must provide proof of meeting the following eligibility requirements:

You have an HIV-positive diagnosis
Your income is within 500% of Federal Poverty Level
You live within the service area.

Even if you are not eligible to be a client, our knowledgeable Care Management staff is always available to answer questions you may have regarding services for people living with HIV/AIDS.
